Chronic left ventricular pseudo-aneurysm after posterior myocardial infarction.

چکیده
DESCRIPTION A 69-year-old man was seen among outpatients after an episode of chest pain. He was pain-free and haemodynamically stable. His electrocardiogram was normal, but a pan-systolic murmur was noted on examination. He was referred for outpatient exercise treadmill testing and transthoracic echocardiography. The echocardiogram demonstrated an akinetic basal posterior wall of the left ventricle that had thinned to the point of rupture (figure 1). Adjacent was a large pseudo-aneurysm measuring 4.5×7.5 cm. Flow across the posterior wall was demonstrated with colour and pulse wave Doppler (figure 2). Subsequent coronary angiography revealed three vessel coronary artery disease with a severe stenosis in the mid-circumflex artery. Rupture of the left ventricular free wall is very uncommon and is almost universally catastrophic. In rare instances the rupture is contained and a pseudo-aneurysm develops. Pseudo-aneurysms usually occur after myocardial infarction, but can occur after surgery or infection.

Establishment of a chronic left ventricular aneurysm model in rabbit
OBJECTIVES To establish a cost-effective and reproducible procedure for induction of chronic left ventricular aneurysm (LVA) in rabbits. METHODS Acute myocardial infarction (AMI) was induced in 35 rabbits via concomitant ligation of the left anterior descending (LAD) coronary artery and the circumflex (Cx) branch at the middle portion. Development of AMI was confirmed by ST segment elevation ...
متن کاملAnterior and posterior left ventricular aneurysm repair concomitant with off pump coronary artery bypass grafting
Background Necrosis after myocardial infarction may cause aneurysm in the left ventricle. Disorders of contractility in the left ventricle due to aneurysm can result with congestive heart failure, ventricular arrhythmia and thromboemboli. This is a retrospective analysis of our clinical experience in the restoration of left ventricular aneurysm via modified linear closure during off pump corona...
متن کاملDiagnosing left ventricular aneurysm from pseudo-aneurysm: a case report and a review in literature
Rupture of the free wall of the left ventricle (LV) is a catastrophic complication occurring in 4% of patients after myocardial infarction (MI) and in 23% of those who die of MI. Rarely the rupture is contained by an adherent pericardium creating a pseudo-aneurysm. This clinical finding calls for emergency surgery. If no ruptures are detectable and myocardium wall integrity is confirmed, we are...
متن کاملPost Myocardial Infarction Submitral Aneurysm Causing Stroke in an Elderly Patient- A Rare Case Report
Left ventricular aneurysms are a frequent and serious complication following acute myocardial infarction and are most commonly located at the ventricular apex. The majority of these patients presents with severe mitral insufficiency, congestive heart failure, systemic embolism and sudden cardiac death. Giant aneurysms occurring in a submitral position between anterior and posterior papillary mu...
متن کاملPrevention of Left Ventricular Aneurysm Formation in Acute Myocardial Infarction
The effect of preexistent coronary collateral perfusion on the prevention of left ventricular aneurysm formation was examined in 47 patients undergoing an intracoronary thrombolysis within 6 hours after the onset of a first acute anterior myocardial infarction. Left ventricular aneurysm formation and wall motion were analyzed with cineventriculography. A left ventricular aneurysm was determined...
